South Carolina, In Detail
What a medical director covers for South Carolina clinics.
South Carolina requires a physician relationship before NPs, PAs, or RNs at a med spa or weight loss clinic can offer medical treatments. The state has no full practice authority, so every NP performing medical acts needs a written practice agreement with a physician. That physician must hold an active, unrestricted South Carolina license and practice in the state. Medical Director Co. matches you with a physician who meets that standard and prepares the paperwork the State Board of Nursing and the Board of Medical Examiners expect.
What slows South Carolina clinics down
RN injectors without on-site coverage
South Carolina board guidance requires a physician or APRN on site whenever an RN injects cosmetic Botox or fillers.
Paperwork that lapses quietly
Practice agreements need a signed annual review, and physician changes must be reported to the Board of Nursing within 15 business days.
Physicians with no open capacity
Each South Carolina physician can work with no more than the equivalent of six full-time NPs and PAs combined, which limits who can take you on.

The Role
How a medical director helps providers in South Carolina.
For NPs, your physician signs the practice agreement that lists the conditions you treat, the drugs you prescribe, and when you refer, then reviews it with you every year. For PAs, the supervising physician sets your scope of practice guidelines, prescribing level, and chart review percentage with the Board of Medical Examiners. For RN injectors, your physician writes the orders and protocols, and a physician or APRN must be on site for cosmetic Botox and fillers.
State Rules
South Carolina medical director and collaborating physician rules.
NP Practice Agreements
Under S.C. Code § 40-33-34, NPs performing medical acts need a written practice agreement with a physician who is readily available in person or by phone or electronic means. The agreement lists the conditions you treat, drugs you can prescribe, and when to refer, and it must be reviewed and signed every year. The Board can request it with 72 hours’ notice, and it audits agreements at random at least every two years.
Prescriptive Authority
NPs can prescribe Schedule III through V drugs listed in their practice agreement. Schedule II non-narcotics are capped at a 30-day supply, and Schedule II narcotics at a 5-day supply unless the patient is in hospice, palliative, or long-term care.
Physician Capacity
A physician can work with up to six full-time-equivalent NPs, PAs, CNMs, and CNSs combined under S.C. Code § 40-47-195. The Board of Medical Examiners can approve exceptions.
PA Supervision
PAs practice under scope of practice guidelines approved by the Board of Medical Examiners. The guidelines set prescribing levels, delegated tasks, and the percentage of charts the supervising physician reviews.
RN Cosmetic Procedures
RNs can inject cosmetic Botox only with a physician or APRN on site, under a joint advisory opinion of three state boards. For dermal fillers, the supervising physician or APRN must be on site and able to respond within five minutes. An RN can’t perform the initial patient assessment; a physician, PA, or APRN must.
Telehealth
APRNs can practice via telehealth under their practice agreement and must meet the same standard of care as in-person visits. They can’t use telehealth alone to prescribe when a diagnosis requires a physical exam.
Compliance mistakes we help you avoid
- RNs injecting Botox or fillers with no physician or APRN on site
- Practice agreements that miss their annual review and signatures
- Changing physicians without notifying the Board within 15 business days
- Prescribing drugs that aren’t listed in the practice agreement
- Signing with a physician who doesn’t actively practice in South Carolina

Common Questions
Frequently asked questions.
Do med spas in South Carolina need a medical director?
Any South Carolina med spa offering injectables, lasers, or prescription treatments needs a physician relationship in place. RN injectors also need a physician or APRN on site during cosmetic Botox and filler treatments, under state board advisory opinions. We set up the agreements and protocols that match your staffing model.
Can nurse practitioners practice independently in South Carolina?
South Carolina still requires NPs who perform medical acts to work under a written practice agreement with a physician. Full practice authority bills (H. 3580 and S. 45) were introduced in the 2025 to 2026 session but haven’t become law. Your agreement must be reviewed and signed at least once a year.
What can NPs prescribe in South Carolina?
NPs can prescribe Schedule III through V drugs when their practice agreement lists them. Schedule II narcotics are capped at a 5-day supply, except for hospice, palliative, and long-term care patients. Schedule II non-narcotics, such as many ADHD medications, allow up to a 30-day supply.
Does my collaborating physician have to be on site?
Your collaborating physician has to be readily available, which South Carolina defines as reachable in person, by phone, or electronically. NP practice agreements don’t require on-site presence. RN-performed cosmetic injections are the exception, since those need a physician or APRN on the premises.
What happens if I need to change collaborating physicians?
South Carolina gives NPs 15 business days to notify the Board of Nursing after changing physicians or practice settings. You’ll also need a new, signed practice agreement to keep performing medical acts. We handle the replacement match and paperwork so your clinic stays open.
How fast can I get a medical director in South Carolina?
Most South Carolina clinics are matched with a licensed physician within 24 hours. A same-day intro call is available, so you can meet your physician before you sign. Your agreements and protocols are drafted as part of the placement.
