Common Hurdles

Three hurdles San Juan clinic owners run into.

01

A smaller physician pool.

Puerto Rico’s physician count fell from about 14,000 to 9,000 between 2006 and 2016, according to Kaiser Health News reporting on PBS NewsHour. Fewer doctors means fewer with time to take on a collaborative agreement.

02

Agreements on file with the board.

Advanced practice nurses submit their collaborative agreements to the Junta Examinadora de Enfermería, so a missing or vague agreement delays your opening.

03

Pharmacy pushback on NP prescriptions.

Puerto Rico’s Pharmacy Law doesn’t list NPs as prescribers yet, and some pharmacies have refused to fill NP prescriptions as a result.

Puerto Rico Rules

Puerto Rico rules your San Juan medical director covers.

NPs

Collaborative agreements for NPs

Law 254-2015 lets advanced practice nurses run a private practice and treat patients through protocols and collaborative agreements with a physician. The agreement spells out the NP’s specific functions and scope, and it goes on file with the Junta Examinadora de Enfermería. We draft yours around the services your clinic actually offers.

Prescribing

Prescribing in Puerto Rico

Under Law 254-2015 and Regulation 9104, NPs may prescribe, including Schedule III through V drugs, in collaboration with a physician. Senate Bill 757 would add NPs to the Pharmacy Law for drugs outside Schedules I and II. The bill, heard in the House on September 2, 2026, would also cap each physician at three collaborative agreements and require review of at least five charts per quarter or 10 percent of active charts, whichever is greater.

PAs

Supervision of médicos asistentes

Law 71-2017 created the médico asistente license, and the medical board began accepting applications in February 2019. A supervising physician co-signs orders and progress notes and can oversee no more than two PAs. PAs in Puerto Rico can’t prescribe medication.

Common Questions

Frequently asked questions.

Puerto Rico requires advanced practice nurses to have a collaborative agreement with a physician. Law 254-2015 ties NP treatment and prescribing to protocols agreed on with that physician. The agreement then goes on file with the Junta Examinadora de Enfermería.

Puerto Rico’s nursing law allows NPs to prescribe under a collaborative agreement, but the Pharmacy Law doesn’t list them as prescribers yet. That gap has led some pharmacies to refuse NP prescriptions. Senate Bill 757 would close it for drugs outside Schedules I and II, and your collaborating physician can prescribe in the meantime.

Botox and dermal fillers are prescription products, so a licensed prescriber has to evaluate each patient and order the treatment. In Puerto Rico, an NP can do that only through a collaborative agreement with a physician. A medical director signs the protocols, standing orders, and agreement your services depend on.

Puerto Rico licenses PAs as médicos asistentes under Law 71-2017. Each one works under continuous supervision by a Puerto Rico-licensed physician, who co-signs orders and notes. PAs can’t prescribe in Puerto Rico, and one physician can supervise no more than two.

Most San Juan placements close within 24 hours of your consultation. You also get a same-day intro call with your matched physician. We draft your collaborative agreement and protocols during the same process.

Medical Director Co. places physicians across Puerto Rico, including Bayamón, Carolina, Guaynabo, Caguas, Ponce, and Arecibo. The same collaborative agreement rules apply island-wide because Law 254-2015 covers all of Puerto Rico. Pricing starts at $799 a month wherever your clinic is located.
