The Opportunity
Why Ohio clinics need collaborating physicians.
Ohio requires certified nurse practitioners, clinical nurse specialists, and certified nurse-midwives to practice under a written standard care arrangement with a collaborating physician (ORC 4723.431). Ohio offers these nurses no independent practice pathway today. Bills that would drop the requirement after 2,000 practice hours (HB 449, HB 508, and SB 258) were still pending in the General Assembly as of October 2026.
Every NP-led med spa, weight loss clinic, and telehealth practice in the state needs a physician to sign on. That’s why medical director jobs in Ohio fit well around a full-time clinical schedule.
Every Ohio placement includes:
- A standard care arrangement or PA supervision agreement drafted to Ohio rules
- Malpractice coverage confirmation before you start
- Clinics matched to your specialty and schedule
- Remote collaboration wherever Ohio law allows it
Common Questions From Physicians
What Ohio physicians ask before signing on.
What exactly am I responsible for?
Each standard care arrangement spells out your duties, including referral criteria, consultation, chart review, and prescribing-pattern review (OAC Chapter 4723-8). We draft it so your role is in writing before you sign.
How many clinics can I take on?
Ohio limits you to five APRNs at once in the prescribing part of your practice and five PAs at any one time. We ask about your current agreements at intake so each new match fits inside those limits.
Who handles the paperwork?
We do. You review the agreement, confirm the terms, and sign.

Standard Care Arrangements
The APRN must have a signed standard care arrangement with you before seeing patients. Ohio doesn’t require Board of Nursing approval in advance, but the agreement must list a recent review date and quality assurance steps. You must stay continuously available by phone, video, or in person.
The Five-APRN Prescribing Limit
You can sign standard care arrangements with any number of APRNs. You can’t collaborate with more than five at the same time in the prescribing component of your practice.
Physician Assistant Supervision
ORC 4730.21 caps supervision at five PAs at any one time. Every supervising physician must also set up a quality assurance system for the PAs they oversee.
Laser and Light-Based Delegation
OAC 4731-18-03 requires on-site supervision when an RN or LPN applies a vascular laser. Starting August 25, 2026, House Bill 377 lets physicians supervise nurses off site for laser hair removal once the nurse completes a 40-hour, physician-approved training course.
Common mistakes we help you avoid
- Signing a standard care arrangement outside your specialty
- Going past five APRNs in prescribing collaboration at once
- Letting agreements sit without a documented review
- Delegating laser procedures at the wrong supervision level

Common Questions
Frequently asked questions.
What do I need to become a medical director in Ohio?
You need an active MD or DO license from the State Medical Board of Ohio. For NP collaboration, your specialty should match or closely relate to the APRN’s specialty. We confirm both at intake before sending any clinic match.
How much time does an Ohio medical director role take?
Your workload depends on clinic volume and the review schedule written into each agreement. Most of the work is chart review, prescribing-pattern review, and staying reachable for consults. We spell out those expectations before you accept a match.
How many clinics can I work with at once in Ohio?
Ohio sets no cap on total standard care arrangements. You can collaborate with up to five APRNs at once in prescribing and supervise up to five PAs at any one time. Several clinics fit within those limits when you plan your matches.
Can I work as a collaborating physician remotely in Ohio?
Ohio lets you meet the availability requirement through electronic communication, so remote collaboration with NPs is common. Some delegated procedures still need you on site, such as vascular laser work done by an RN or LPN. We match you with clinics whose services fit how you want to work.
How does Medical Director Co. handle malpractice coverage?
We confirm malpractice coverage for every Ohio placement before the collaboration starts. You’ll see where coverage stands before you sign anything. Ask us at intake how coverage applies to your specific agreement.
What happens if I want to end a collaboration agreement?
Give the APRN written or electronic notice to end the standard care arrangement. The APRN may keep practicing under it for up to 120 days after notifying the Board of Nursing. Add your preferred notice period to the agreement so both sides know the timeline.
