How Ohio's Collaboration Rules Shape Gahanna Clinics
Gahanna is a Franklin County city of 35,726 residents (2020 Census) on the northeast edge of Columbus, and its clinics follow Ohio’s collaboration rules. Ohio nurse practitioners need a Standard Care Arrangement (SCA) with a collaborating physician for as long as they practice, since the state has no path to independent NP practice. Medspas also need a physician directing care, because Ohio treats neurotoxins, fillers, and medical lasers as the practice of medicine. In Gahanna, your medical director or collaborating physician signs the agreement Ohio requires for your license: an SCA for NPs, a supervision agreement for PAs, or written orders and protocols for RNs. That physician also sets your prescribing terms and signs off on which procedures each provider can perform. Ohio NPs need a collaborating physician for their whole career, so this relationship matters as much for veteran NPs as it does for new grads.
Ready to get matched with an Ohio-licensed physician?
Common Hurdles Gahanna Clinics Face
Ohio requires your collaborating physician to practice in a specialty that’s the same as or similar to yours, so an aesthetics NP can’t sign with just any available doctor.
An Ohio physician can collaborate with no more than five APRNs at once in the prescribing part of their practice, so a physician already at that limit can’t take you on.
If your collaborating physician ends the arrangement, you can keep practicing under your existing SCA for only 120 days after you submit the termination notice to the Ohio Board of Nursing.

Compliance Deep-Dive
Ohio Rules for Hiring a Medical Director in Gahanna, OH
ORC 4723.431 requires every certified nurse practitioner, certified nurse-midwife, and clinical nurse specialist to practice under a written SCA with each collaborating physician. The Ohio Board of Nursing doesn’t pre-approve the SCA, but it can review it for compliance. You must report each collaborator’s name and business address to the Board within 30 days of starting practice, and report later changes within the same window.
Ohio APRNs prescribe within the terms of their SCA, and their authority can’t exceed the collaborating physician’s. Schedule III through V drugs are generally allowed, while ORC 4723.481 limits Schedule II prescribing to narrow situations and bars it from convenience care clinics. Your SCA should also cover how you check OARRS, the Ohio Automated Rx Reporting System.
PAs practice under a supervision agreement with each supervising physician, who accepts legal responsibility for the PA’s services. The State Medical Board of Ohio stopped filing these agreements in 2018, so yours stays in your records for inspection. One physician can supervise up to five PAs at a time under ORC 4730.21, and a PA’s first 500 hours of prescribing require on-site supervision under OAC 4730-2-04.
Ohio RNs can administer Botox, fillers, and IV therapy with a valid order from an authorized provider, but they can’t select the drug or the dose. The Ohio Board of Nursing expects documented education and competency for each procedure. Delegation of medical tasks to unlicensed staff falls under OAC Chapter 4731-23, and estheticians can’t inject neurotoxins or fillers in Ohio.
House Bill 377, effective August 25, 2026, updated Ohio’s laser hair removal law, ORC 4731.33. Physicians can now supervise up to five delegates at once, up from two, and can supervise nurses off-site after the nurse completes a 40-hour physician-approved course. APRNs can delegate and supervise laser hair removal if their SCA doesn’t prohibit it, and PAs can with an active supervision agreement. Other light-based procedures still follow OAC Chapter 4731-18.
Ohio’s telehealth statute, ORC 4743.09, requires each licensing board to let its professionals provide services through telehealth, and it leaves NP and PA collaboration and supervision rules unchanged. The State Medical Board’s rule, OAC 4731-37-01, covers care for patients located in Ohio, and controlled substance prescribing through telehealth must fit a federal exception.
The State Medical Board of Ohio’s position statement on the corporate practice of medicine says the doctrine no longer exists in Ohio, so an NP, RN, or business owner can own a Gahanna medspa as long as a physician directs the medical care. The two exceptions are professional associations, which only licensed professionals can own, and pain management clinics, which only physicians can own and operate.
Common Compliance Mistakes We Prevent
- NPs practicing without a current SCA or with a collaborator outside their specialty
- Missing the 30-day Board of Nursing notice after adding or changing a collaborator
- RNs injecting without a documented order or competency record
- Delegating laser hair removal to staff who don't qualify under ORC 4731.33
- PA supervision agreements that no longer list every supervised PA

FAQs
FAQs: Gahanna, OH Medical Director
Ohio treats Botox, fillers, and medical lasers as the practice of medicine, so a Gahanna medspa needs a physician directing that care. Ohio doesn’t issue a separate medspa license. Your medical director writes the protocols and orders your staff work under.
Ohio NPs can own and open a clinic, but they need an SCA with a collaborating physician before seeing patients. The physician must practice in a specialty that’s the same as or similar to the NP’s. The NP then reports that physician to the Ohio Board of Nursing within 30 days.
Ohio RNs administer injectables and IV therapy under a valid order from an authorized provider, such as a physician or APRN. The RN can’t choose the drug or the dose. The Ohio Board of Nursing also expects documented training and competency for each procedure.
Ohio’s telehealth law, ORC 4743.09, lets Ohio-licensed physicians provide services remotely. Remote work doesn’t change the collaboration or supervision terms in your agreement. PAs are the main exception, since their first 500 hours of prescribing require on-site supervision.
The State Medical Board of Ohio says the corporate practice of medicine doctrine no longer exists in the state. A non-physician can own a medspa, provided a physician directs all medical care. Pain management clinics are the exception and must be physician-owned.
ORC 4723.431 caps each physician at five APRNs at the same time in the prescribing part of their practice. A physician can sign more SCAs than that, but only five can involve active prescribing collaboration at once. Ask about a physician’s current APRN count before you sign.
